Many years ago already, the anatomy of root canals was described as showing variability and being of a complex nature. According to Vertucci, the mandibular first premolar has one canal at the apex in 74.0% of teeth, two canals at the apex in 25.5% and three canals at the apex in 0.5% of teeth.

Periodontal disease is still a key area of interest in dentistry today. While, in the past, we relied on improving treatments in technical and material terms, with today’s possibilities, treatments that enable or guarantee long-term success are being sought. The hard and soft tissue surrounding the tooth or implant unfortunately continue to be a weak point in ongoing treatment with any kind of prostheses whatsoever.

My learning pathway with the IAS Academy was slightly different from the normal route in that I took the ClearSmile Aligner course first, then the ClearSmile Inman Aligner one and then the ClearSmile Brace course. I trained with aligners first by shadowing my former principal, who used a different clear aligner system to great effect.

Whether we like it or not, we are embracing the digital era in our brave new world. Many dental practices are now becoming paper-free—a digital innovation—and even using tablet computers to record patient details and medical histories. We are continually surprised by the rising age of the technologically savvy patient, particularly those of a certain generation who perhaps we assume would be less so than the perceived iPhone generation.

The patient reported on in this article is a student in dentistry and his parents are both dentists. They referred their son to a good endodontist, who then referred the case to me. As always, peers are more than welcome in either of my practices, in Rome and London, so when I treated this case, I had three dentists watching me, a future dentist on the chair, placing a great deal of pressure on me.

The need for an endodontic skill set within the profession is higher than it has ever been. Culturally and socially, there have been significant changes in patient perception to the news that a tooth is in trouble, where the solution is either root canal treatment or removal. I am old enough to have treated patients who needed the replacement of complete dentures that were a “wedding gift” in their youth. As a gesture of goodwill to the bride in waiting, wholesale extractions and the provision of complete dentures were gifted to ensure the absence of dental problems or expense throughout their new found love.

Lee—who has been the subject of much controversy in recent months. These two men did however have something in common: a belief in the importance of lifelong learning. Einstein said: “intellectual growth should commence at birth and cease only at death,” while Lee declared, “the education of a man is never completed until he dies”.

In an article by business author Jaron Lanier in the Jan/Feb 2018 edition of Wired UK magazine, he reminds us that Facebook and Google now account for 75 per cent of online advertising spend, globally. In the same article, there is also a fascinating quote about the current problems with the Internet.
